Iwould like to to obtain a letter format asking businesses to donate money or goods to a worthy local cause?

I work as a volunteer for the ambulance service as a first responder, and although we are trained and kitted by them we require funds to keep going, I have been tasked to approach companies for donations, but do not know the best way to go, any ideas please.
